This position is eligible for a $10,000 retention bonus.\n\nJob Summary\n\nThe Providence Center Child and Family Services Healthy Transitions Registered Nurse (RN) will provide nursing services for ages 16 -25 with mental health and / or co-occurring disorders, which serves clients and their families in the community. Will assess, plan, develop, coordinate, and provide treatment and supportive services to children with mental health issues and their families.\n\nServices will be provided in the community.\n\nCoordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage consumers to attain the highest possible levels of independence through providing outpatient psychotherapy either at The Center or in the community.\n\nThe Healthy Transitions Nurse must have professional knowledge and understanding of the characteristics of children and young adults with mental health issues, their families, and family systems. Knowledge of community resources or ability to learn quickly as well as good organizational, time management and communication skills. Ability to work well with community partners and in a team environment is essential. Highly motivated, self-directed, willing to work a variety of hours, including evening and on-call.\n\nSchedule : Monday through Friday 8 : 30 am – 4 : 30 pm with an half an hour for lunch. This position also requires some evening hours.\n\nDuties And Responsibilities\n\nPerform psychiatric nursing in accordance with appropriate medical protocol.\n\nProvide accurate and timely information about medications and their side effects to the client and relevant others.\n\nUse medication maintenance strategies, determining that the doctor's orders have been accurately transcribed, implementing appropriately the prescribed medical regimen, monitoring lab results to validate medication compliance and assessing any aspect of the consumer's treatment regimen that endangers the level of wellness, communicating questionable aspects of consumer care to appropriate persons.\n\nUse physical assessment skills to determine related nursing interventions, including when medication exacerbates medical conditions and recognition of basic medical problems.\n\nInform and educate clients about mental illness, medication, coping skills, their rights as clients, services, community resources, self-help and the consumer movement.\n\nUtilize best practices of intervention and support strategies.\n\nTeaches both simple and complex skills, including physical, social, cognitive emotional, and other relevant skills.\n\nIncorporate consumer choice in treatment.\n\nMake period reassessments and if warranted, change treatment plans.\n\nDemonstrate appropriate use of range of crisis prevention and intervention strategies, including crisis prevention and intervention, risk assessment, hospital alternative, hospitalization, and assessment of self care, performing calmly and professionally during crisis or emergency situations.\n\nCoordinate activities regarding consumer hospitalization, serving as liaison with the hospital regarding admission, case review, and discharge.\n\nComply with written clinical procedures and professional standards.\n\nAddress the unique needs of individuals and co-occurring disorders (e.g., substance abuse, developmental and / or physical disabilities, personality disorders, trauma, brain injury, etc.) that consumers have.\n\nKeep current about new developments in psychotropic medication and other biological interventions.\n\nMake assessments and provide appropriate crisis interventions and risk management as needed.\n\nTeach consumers adult daily living skills.\n\nInitiate treatment and advocate for consumer within network of care.\n\nNegotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.\n\nTransport consumers in own vehicle or TPC van to planned activities and appointments.\n\nComplete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by Center confidentiality policy.\n\nMaintain cooperative relationships with other employees, consumers and the public.\n\nAttend trainings and conferences.\n\nServe on appropriate Center committees.\n\nRequirements\n\nAssociate's degree in Nursing required; Bachelor's degree from accredited four-year College or University preferred.\n\nRhode Island Registered Nurse (RN) license or Compact License required.\n\nExperience In Human Services Or Outpatient Setting Preferred.\n\nExperience working with psychiatric and substance use disorder patients also a plus.\n\nBilingual Spanish preferred.\n\nMust have valid driver's license and insured auto.\n\nMust be able to work flexible hours.\n\nCare New England Health System (CNE) and its member institutions;Butler Hospital, Women & Infants Hospital, Kent Hospital, VNA of Care New England, Integra, The Providence Center, and Care New England Medical Group, and our Wellness Center, are trusted organizations fueling the latest advances in medical research, attracting top specialty-trained doctors, and honing renowned services and innovative programs to engage in the important discussions people need to have about their health.\n\nAmericans with Disability Act Statement : External and internal applicants, as well as position incumbents who become disabled must be able to perform the essential job-specific functions either unaided or with the assistance of a reasonable accommodation, to be determined by the organization on a case-by-case basis.\n\nEEOC Statement : Care New England is an equal opportunity employer.
