Pharmacist - In-Patient Pharmacy - PRN

Requirements :

What to expect as an In-Patient Pharmacist :

• Ensure safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, and dose / therapy adjustment as applicable. Document medication-related interventions in medical record.
• Provide clinical support to practitioners. Make recommendations and encourage safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
• Oversee the medication dispensing process to ensure accurate and timely delivery of medications to patients. This includes supervision of pharmacy technicians and / or interns.
• Oversee sterile compounding activities.
• Perform other duties as assigned.
