Clinical Pharmacist

Description

Clinical Pharmacist at St. Joseph Hospital Eureka, CA. This position is Per Diem and will work 10-hour, Evening shifts.

The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.

Required Qualifications :

• Pharmacy Doctorate, or
• Bachelor's Degree in Pharmacy
• Upon hire : California Pharmacist Registered
• When eligible and where applicable, obtain a Board of Pharmacy Preceptor license
• Where applicable Basic Life Support-Provider (BLS)
