Description :

This position requires a Physical Therapist for home health services.

The shift is a regular day shift, working five 8-hour days.

Call is required once every five weeks on Sundays.

Every fifth week, the therapist will take two days off during the week and work on Saturday and Sunday to complete admissions.

Candidates must possess required certifications : BLS, DPT, and a state license.

A minimum of one year of experience is required.

The role covers various counties, with scheduling that rotates weekly for visits in the furthest areas.

When mileage and travel times are significant, fewer patients will be seen that day.

The physical therapist will handle admissions, evaluations, re-assessments, and discharges.

The majority of diagnoses will involve general weakness, back pain, and orthopedic conditions.

Physical Therapy Assistants will manage routine visits.

Dress code requires scrubs in black, grey, or khaki pants with a polo or scrub top.

Adaptive devices and treatment modalities are essential skills.

Experience with Epic charting system is preferred.

This position will involve collaborating with interdisciplinary support teams.

Candidates should have experience with patient education and rehabilitation for various conditions.

All candidates must comply with required vaccinations, with limited exemptions available.
