Compact RN Nurse Care Manager

A company is looking for a Newborn / Pediatric Nurse Care Manager who is passionate about providing holistic care to members through their healthcare journey.

Key Responsibilities :

Deliver coordinated, patient-centered virtual Care Management via telephone and video to improve health outcomes

Create impactful care plans with members and the care team to achieve desired health goals

Assist members in navigating complex medical conditions, treatment pathways, and the healthcare system

Required Qualifications :

Bachelor of Science in Nursing (BSN)

Current CCM Certification or eligibility within 6 months of hire

5+ years of nursing experience, with 2+ years in care management

Active Compact RN license in good standing

Must reside in a compact NLC state
