Annual Salary : $80k-$105k (varies depending on years of experience)A reputable human services organization in Westchester County is seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality healthcare support to adults with intellectual and developmental disabilities living in residential settings. The RN plays a vital role in promoting wellness, coordinating medical care, and ensuring that individuals receive comprehensive, person-centered services in accordance with regulatory standards.
- Key Responsibilities
- Deliver and monitor nursing care and medication administration as prescribed by physicians.
- Conduct regular visits to assigned residential programs to evaluate health status, review care needs, and ensure compliance with nursing protocols.
- Develop and update individualized care plans based on each persons medical and behavioral needs.
- Serve as a liaison among families, healthcare providers, and internal teams to ensure clear communication and coordinated care.
- Provide instruction, guidance, and oversight to Direct Support Professionals (DSPs) and Approved Medication Administration Personnel (AMAPs).
- Oversee AMAP training and recertification, including medication administration, G-tube, and diabetes management.
- Maintain accurate and timely documentation in the agencys electronic health record system.
- Write Plans of Nursing Services (PONS) and other required nursing documentation in compliance with OPWDD and agency policies.
- Participate in interdisciplinary team (IDT) meetings and contribute to ongoing assessments and treatment planning.
- Support the scheduling and tracking of medical appointments, consultations, and follow-ups.
- Ensure health and safety standards are met in all assigned residences and promptly report any concerns.
- Stay current with continuing education, nursing best practices, and state-mandated training requirements.
- Provide on-call coverage and emergency nursing support as needed.
- Collaborate regularly with the Director of Nursing and other members of the clinical team.
- Attend and complete required agency and OPWDD trainings.
- Perform additional duties as assigned to support the overall mission of the organization.
- Qualifications
- Licensed and registered by the New York State Education Department as a Registered Nurse.
- Minimum of one year of nursing experience working with individuals with intellectual and / or developmental disabilities (ICF / IRA experience preferred).
- Strong organizational, written, and verbal communication skills.
- Proficiency in electronic medical record systems and general computer use.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Demonstrated commitment to person-centered care and advocacy for individuals served.
