Facility Overview

Responsibilities
Perform nursing assessments within 8 hours of admission to create individualized care plans.Identify patient / family teaching needs and collaborate with the interdisciplinary team.Provide timely updates for Team Conferences (initial and weekly re-evaluation).Accurately document all assessments, interventions, and patient responses per policy.Deliver direct patient care using the nursing process (assessment, planning, implementation, evaluation).Educate patients / families to support rehab goals and discharge readiness.Initiate discharge planning from admission.Communicate patient status changes promptly to physicians and charge nurse.Supervise and delegate tasks to LPNs, CNAs, and ancillary staff.Ensure personal and environmental safety, following hospital guidelines.Contribute positively to team morale and provide calm leadership.Monitor supplies, promote cost-effective care, and escalate equipment / service needs.Assume charge nurse responsibilities as needed.Engage in ongoing professional development.Perform other duties as assigned.Requirements
Required Certifications :BLS
ACLSExperience :Minimum 2 years bedside clinical experience (highly desired)
