We value contributions from everyone involved in patient care.Your Role as a Hospice Nurse :
Conduct initial assessments of patients’ physical and psycho-social status.Collaborate with the Interdisciplinary Group (IDG), patients, families, and attending physicians to develop personalized care plans.Provide education and interventions to effectively manage pain and other symptoms, while assessing responses to treatment.Ensure all necessary equipment, supplies, and medications are readily available.Adapt care plans as patients’ and families’ needs evolve.Document patient and family services accurately and promptly.Participate in regular IDG meetings to enhance patient care.What We’re Looking For :
Registered Nurse : Must hold an active, unrestricted license in the State of Michigan.Experience : Community health and a broad range of Medical-Surgical experience is preferred.Transportation : Reliable transportation with a valid state driver's license and current car insurance.At Interim HealthCare, we are proud to be an Equal Opportunity Employer.
