Essential Duties and Responsibilities :
~ Prepare medications by reviewing and assessing orders to identify, prevent and resolve drug related problems.
~ Provide accurate and understandable counseling and education to clients and staff.
~ Perform drug utilization reviews and check for drug interactions when filling every prescription.
~ Maintain clinical hours each week outside of the pharmacy to be available for clinical team meetings and discussions as necessary.
~ Provide oversite to pharmacy staff at assigned locations and serve as main point of contact for all site-based pharmacy concerns.
~ Perform monthly audits of assigned location, including the review of documentation,340B compliance, proper inventory management, and other areas as assigned by Regional Pharmacy Manager.
~ Communicate effectively with doctors, nurses, and other providers and leadership members.
~ Prepare and report from assigned location to be sent to the Regional Pharmacy Manager monthly, often as required.
~ Interview and hire all site-based pharmacy staff with assistance from the Regional Pharmacy Manager.
~ Always maintain patient rights and confidentiality.
~ Answer questions on drug therapy when appropriate.
~ Communicate with other offices regarding refills and other medication issues.
~ Work with Suboxone coordinator to provide dispensing to clients.
~ Notify and collaborate with Human Resources on staffing issues including but not limited to policy and procedures, employee handbook, disciplinary issues, ADA, FMLA, harassment, workers compensation, leaves of absence, legal issues, orientation, internal transfers, training of new and current staff.
~ Work collaboratively with Leadership Teams and Clinical Teams.
~ Complete other tasks as assigned from time to time.
~ Decision Making Authority : Responsible for pharmacy leadership of the assigned location, under the guidance of the Regional Pharmacy manager, as well as daily pharmacy duties. All other requirements to be met with advisement from the COO, operations and clinical leadership teams, as required.
Valid Indiana Pharmacist License, active and in good standing. Ten-year experience as a pharmacist preferred. American Heart Association Basic Life Support certified required.
