We are currently seeking a licensed and compassionate Speech-Language Pathologist (SLP) to join our growing home health care team.

Position Summary :

As a Home Health Speech-Language Pathologist, you will play a vital role in helping patients regain their communication, cognitive-linguistic, and swallowing abilities after illness or injury. You will conduct in-home evaluations and develop individualized treatment plans tailored to each patient's needs. All documentation is completed using the Net Health EMR system to ensure accuracy and regulatory compliance.

Key Responsibilities :

Perform in-home assessments to evaluate speech, language, voice, cognition, and swallowing

Develop and implement individualized treatment plans to address patient goals

Provide direct speech-language therapy services in the home environment

Educate patients and caregivers on communication techniques and home programs

Document all evaluations, care plans, and progress notes in Net Health EMR system

Collaborate with physical therapists, occupational therapists, nurses, and physicians to provide holistic care

Ensure care is delivered in compliance with home health regulations and agency standards

Qualifications :

Master’s degree in Speech-Language Pathology from an accredited program

Current and unrestricted Pennsylvania SLP license

Certificate of Clinical Competence (CCC-SLP) from ASHA (or CFY candidates with supervision considered)

CPR certification (or willingness to obtain)

Valid driver’s license and reliable transportation

Home health experience preferred but not required – training provided

Familiarity with EMRs; Net Health experience is a plus

Strong interpersonal, time management, and communication skills
