About the Role :
We’re seeking a dedicated Nurse Practitioner to join our Salem, Oregon care team. You’ll provide a blend of clinic-based and in-home services within a 30-mile radius, working closely with high-risk patients in need of continuity and proactive medical attention. This is a full-time, weekday role focused on relationship-building, preventive health, and effective chronic disease management.
What You'll Do :
- Conduct comprehensive patient assessments, both in clinic and home-based settings
- Perform annual wellness visits, risk adjustment coding, and medical reconciliation
- Monitor and manage chronic conditions through evidence-based protocols
- Collaborate with interdisciplinary team members including social workers and care coordinators
- Support patient transitions following hospital discharges and develop care plans to reduce readmission risk
- Utilize EMR systems for timely, accurate documentation (72-hour turnaround expected)
- Participate in care team meetings to enhance holistic treatment approaches
- Provide high-touch, patient-centric education on diagnoses, medications, and long-term care planning
- Rotate coverage with other NPs as needed to ensure consistent access for patients
- Maintain confidentiality in accordance with HIPAA and uphold documentation best practices
- Expected daily caseload :
- Clinic : ~10–12 patients
- In-Home : ~5–6 patients
Qualifications :
Current and unrestricted Nurse Practitioner license in OregonBoard certification (FNP, AGNP, or related) through ANCC or AANPMinimum of 2 years’ experience in adult / geriatric care, primary care, internal medicine, or related fieldComfortable working with complex patients in home and outpatient environmentsFamiliarity with Medicare populations, risk-based care, and HCC documentationActive DEA registration or willingness to obtainValid Oregon driver’s licenseCPR / BLS certificationHome health or transitional care experience is a plusTraits for Success :
