Job Description :

Sunbelt Staffing is seeking a skilled Home Health RN to join a reputable agency in Rocky Mount, NC for a 13-week travel contract. This role involves providing in-home skilled nursing care, completing OASIS assessments, and managing patient care plans. Candidates must be experienced in home health nursing and proficient with Homecare Homebase documentation.

Key Responsibilities :

Conduct in-home skilled nursing visits

Complete OASIS start-of-care, recertification, and discharge documentation

Develop and update individualized care plans

Communicate with physicians and interdisciplinary care teams

Educate patients and caregivers on treatment plans and medication management

Document all care and patient interactions using Homecare Homebase

Qualifications :

Active RN license in North Carolina

Minimum 2 years of recent home health nursing experience

Proficiency with OASIS documentation (required)

Experience with Homecare Homebase (required)

Current BLS certification

Reliable transportation, valid drivers license, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
