Description :

Seeking an experienced Home Health RN with Certified Home Health experience and proficiency in EPIC or Netsmart.

Responsibilities include developing and implementing patient care and discharge plans, performing OASIS assessments, coordinating with physicians and care teams, supervising LPNs, and ensuring quality and compliance with state and federal regulations.

Strong patient care, communication, and clinical skills required.

Required :

RN License (NY License ONLY)

BLS

Certified Home Health Care Experience Required

One year of experience as a clinical nurse in a hospital required, and 9 months experience in home care preferred

EPIC or Netsmart Required

Oasis Experience Required

Telemetry Experience Required
