Position Summary
We are seeking a dedicated and mission-aligned Advanced Practice Provider to join our integrated care team. The Advanced Practice Provider will provide high-quality, age-appropriate, and specialty-specific care to LLCHC outpatients in coordination with a collaborating physician. The APP will practice within their scope of licensure, certification, and education, and will work alongside a multidisciplinary team to deliver holistic care in alignment with LLCHC's faith-based mission.

Key Responsibilities
- Deliver high-quality, patient-centered primary care to individuals and families of all ages.
- Manage clinical schedules efficiently while maintaining patient access and continuity of care.
- Utilize evidence-based guidelines to support diagnostic and treatment decisions.
- Document care in a timely, accurate, and compliant manner within the EHR (Epic experience preferred).
- Serve as a clinical leader in your care team and actively participate in team-based care delivery.
- Support the implementation of clinical initiatives that improve patient outcomes and health equity.
- Participate in quality improvement efforts and contribute to a culture of excellence and accountability.

Primarily clinic-based; may include occasional community outreach or telehealth.Ability to lift up to 25 lbs, bend, stoop, and move intermittently throughout the day.May require occasional evening or weekend hours based on patient needs.Medical care will be of the highest quality and will be monitored by peer review and continuous quality improvement.Required Qualifications
Master's or Doctoral degree in Nursing or Physician Assistant StudiesBoard certification in the relevant specialty2+ years of clinical experience in a healthcare settingStrong communication and interpersonal skillsExperience with telemedicine servicesCurrent and unrestricted state license to practice as an NP or PANational certification from recognized certifying bodies (e.g., AANP, ANCC, NCCPA)Current BLS certification (ACLS and PALS preferred).Demonstrated commitment to serving diverse, underserved, or vulnerable populations.Strong interpersonal and communication skills with a team-oriented approach.Preferred Qualifications
