Ability to assess patients and organize family meetings is essential.

Competency in arranging DME, SNF, and ambulance transport must be demonstrated.

Proficient in computerized documentation and Microsoft applications is required.

Travel experience is necessary for this position.

The ability to evaluate treatment plans is essential.

Candidates must be skilled in utilizing interqual criteria.

Organizational skills must be strong.

Candidates must participate in the development of treatment plans.

Experience in handling diverse patient demographics is expected.

Ability to navigate protective services processes is necessary.

Experience with patients experiencing homelessness or mental health issues is required.

Strong communication skills are essential for interacting with interdisciplinary teams.

Knowledge of CMS and DRG is a must.
