What You’ll Be Doing

Lead Patient Intakes : Collect and organize referral and medical documentation with precision.

Coordinate With Providers : Work closely with doctors, nurses, and care teams to ensure seamless care.

Handle Insurance & Authorizations : Verify coverage and secure approvals promptly to avoid delays.

Maintain Accurate Records : Enter and manage patient information in EMRs and digital systems.

Stay Compliant : Uphold HIPAA and privacy standards at all times.

Support Patients & Families : Communicate clearly, professionally, and compassionately.

What You Bring

Education : High school diploma required; associate’s or bachelor’s preferred.

Experience : Minimum 2 years in healthcare — Home Health / Hospice intake experience required.

Skills & Knowledge : Proficient in EMR systems, medical terminology, and insurance procedures.

Detail-Oriented : Catch what others miss and keep processes running smoothly.

Strong Communicator : Confident, compassionate, and effective with patients, families, and providers.
