We are seeking a dedicated and skilled Advanced Practice Provider (Board Certified Nurse Practitioner or Certified Physician Assistant) to join our healthcare team. The ideal candidate will play a crucial role in delivering high-quality patient care in a post-acute care setting.
Duties
- Conduct thorough patient assessments and develop individualized care plans.
- Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
- Administer treatments and procedures as per established protocols.
- Maintain accurate medical records and documentation in compliance with medical terminology standards.
- Stay updated on the latest advancements in physiology knowledge and trauma care practices.
- Participate in quality improvement initiatives and contribute to the development of best practices within the facility.
