Title : Mercy Housing Residential Engagement Coordinator

Status : Full Time

Reports to : Director of Care Coordination

Purpose :

The Residential Engagement Coordinator will be responsible for facilitation of C4 service delivery between Mercy Housing and C4 connecting prospective clients to desired service(s) for the purpose of assisting Mercy Housing residents to maintain housing. Establishing and maintaining fluid and transparent relationships between Mercy Housing and C4. The ideal candidate will possess high level networking ability, be a skilled resident advocate, well organized and able to problem solve. Minimum Qualifications :

1. Bachelor’s degree in social work or related field from an accredited university required.

2. Master’s degree in social work, Psychology, Sociology, or an applicable human service field from an accredited university preferred.

3. Minimum three (3) years’ experience in healthcare administration / management with experience in operations management, and / or finance preferred.

4. Knowledge of federal, state, CMS, Medicaid and various licensing, certification and accreditation requirements.

5. An equivalent combination of academic, clinical and operational experience may be considered

6. Current, valid, and active driver’s license required

Competencies (Minimum Skills, Knowledge and Experience) :

Effective communication skills both oral and written
The ability to utilize Microsoft Office tools; Teams, Word, Excel, SharePoint, and PowerPoint
Familiarity with utilization of databases, EHR platforms
Experience with program development and project management
The ability to maintain and accurately document engagement using the DAP method and critical incident report standards
Adhere to deadlines and prioritize assigned work and projects
The ability to assess co-occurring disorders
Understand the practice of harm reduction
Utilize a person-centered approach to care
Understand and utilize cultural humility in service provision
Understand the Social Determinants of Health relative to client population needs
Awareness and utilization of Motivational Interviewing
Understanding of personal and professional boundaries

Duties and Responsibilities :

Develop and lead various group activities at the Art Studio which involve simple art and craft-making activities to engage residents and build trust

Develop and lead psychoeducational groups at housing provider sites, encouraging residents to discuss topics such things as : social skills, coping skills, anger management, PTSD, anxiety, depression, etc.

Actively engage residents and encourage use of the Art Studio and participation in scheduled activities

Maintain a monthly schedule of groups and activities in the Art Studio

Collaborate with Mercy Housing staff regarding resident’s provision of care on continuous basis to ensure quality services, without service duplication or disruption

Apply appropriate trauma-informed interventions

Utilize Motivational Interviewing to encourage and support residents who may benefit from behavioral health services

Coordinate with housing provider case management staff and property management to support clients identified needs and help ensure stable housing.

Determine appropriate type and level of services

Provide culturally appropriate, competent, and individualized linkage plans in accordance with each client’s age, gender identity, sexual orientation, race, ethnicity, religious beliefs, and culture

Identify need for crisis intervention when indicated working collaboratively with C4’s Mobile Crisis Team.

Aid residents in maintaining housing

Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and surrounding communities

Submit weekly reports as requested by supervisor

Meet weekly with assigned clinical supervisor and actively seek additional consultation whenever necessary or requested by supervisor

Maintain compliance with confidentiality policies and procedures

Attend key staff and resident meetings to promote use of the Art Studio and other on-site services

Other duties as assigned by supervisor
