Remote Registered Nurse Care Manager

A company is looking for a Care Manager, Registered Nurse- Remote.

Key Responsibilities

Engage with members, caregivers, and providers to develop and implement comprehensive care plans

Assess ongoing care needs and adjust care plans based on member progress and changing conditions

Coordinate patient education and facilitate referrals to community resources based on member needs

Required Qualifications

Current multi-state compact Registered Nurse licensure; BSN preferred

3-5 years of varied clinical experience, with a preference for telephonic Case Management experience

Training in motivational interviewing preferred

Proficiency in electronic medical records and Microsoft Office applications

Ability to work from a home office that meets HIPAA requirements
