Utilizing the Medical Model and Nursing Process (Assessment, Panning, Nursing Diagnosis, Implementation and Evaluation) provides, and documents care to nephrology in accordance with the medical plan of care, critical pathways, established Self Regional Healthcare, Self Medical Group, and Medical / Nursing Policies / Procedures / Protocols and Standards of Care. Coordinates and delegates the care with / to other professionals and ancillary personnel. Communicates findings as indicated to the Nephrologist. Provide family centered care and acts as a clinical resource person to nursing and medical staff. Performs any other assigned duties per Nephrologist, VP of Medical Affairs, Practice Manager, and Self Medical Group Administration. Must be licensed to practice as an Advanced Practiced Register Nurse (APRN) in South Carolina, must be certified
as a Nephrology Nurse Practitioner (NNP). Must have prescriptive authority in South Carolina along with Federal and SC DEA Licensure. BLS and Nephrology program certifications required.
Graduate of an accredited school of medicine and approved program for Nephrology Nurse Practitioner, certification as a Nephrology Nurse Practitioner, current licensure, clinical expertise, leadership, and organizational ability. Must be able to communicate effectively and work collaboratively with others. Must be self-motivated. Must have significant practice experience in private practice or institutional setting.
