Job Title : Nurse Practitioner – Remote / In-Field
Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM (1-hour lunch)
On-Call : Rotation every 6–7 weeks with additional compensation
About the Role
The Nurse Practitioner will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. This position offers the opportunity to make a significant impact on patient outcomes and promote continuity of care. Patient visits will be completed in-home and / or in community-based settings such as assisted living facilities, skilled nursing facilities, group homes, or other delegated locations.
Must-Have Qualifications
Active Advanced Practice Provider (APP) License
DEA License
Minimum 1 year of prior experience as an APP

Essential Functions
In-Home Provider Visits : Conduct a minimum of 25 comprehensive in-home assessments per week, including medical history reviews, physical exams, and medication management, within assigned territory.
Patient Assessment : Perform thorough evaluations of patients’ health status, including histories, physicals, and risk assessments.
Diagnosis & Treatment : Develop accurate diagnoses and individualized treatment plans based on evidence-based practices.
Chronic Disease Management : Monitor and manage chronic conditions (e.g., diabetes, hypertension, cardiovascular disease) while providing education to improve control and outcomes.
Medication Management : Prescribe and adjust medications as appropriate while educating patients on safe use and side effects.
Preventive Care : Promote preventive healthcare measures, such as screenings, immunizations, and lifestyle modifications.
Patient Education : Provide education on disease management, self-care, and healthy living to empower patient involvement.
Collaborative Care : Work closely with physicians, nurses, and other professionals to ensure coordinated and seamless care.
Primary Care Role : Serve as the primary care provider of record for assigned patients.
Documentation : Accurately complete EMR documentation within 48 hours of visits.
Bedside Manner : Foster trust and rapport through empathy and patient-centered communication.
Follow-Up Care : Conduct ongoing follow-up visits to monitor treatment effectiveness and adjust as needed.
Quality Assurance : Participate in quality initiatives and adhere to care standards and regulations.
After-Hours On-Call : Provide longitudinal care and IHA visits during periodic after-hours call rotations.
Travel : Maintain reliable transportation for patient visits within assigned territories.
Compliance : Adhere to all policies, procedures, codes of conduct, and compliance standards.
Confidentiality : Comply with federal, state, and local privacy / security regulations.
Other Duties : Perform additional responsibilities as assigned based on business needs.
