Job Duties :
Provide primary care rounding in SNF and ALF settingsDiagnose and manage acute and chronic medical conditionsDevelop and implement individualized care plansConduct clinical case reviews and share best practicesFoster strong relationships with patients, families, and facility staffCompensation & Benefits :
