Job Summary
The CSW provides comprehensive assessment, case management / brokerage, individual therapy and crisis intervention services to a caseload of approx. 15 clients. Their work is done both onsite and in the community.
Department Description
The Emergency Department Case Management (EDCM) program is an out-patient, intensive clinical case management program within the UCSF Dept of Psychiatry at Zuckerberg San Francisco General Hospital. EDCM facilitates psychosocial, medical and psychiatric care by establishing therapeutic relationships with individuals who experience complex medical and behavioral health issues and frequently rely on the Medical Emergency Department and / or on Inpatient medical services at ZSFG. EDCM clients are generally unhoused, diagnosed with severe mental illness, substance using, medically fragile with complex trauma histories. To provide person-centered care, the interdisciplinary team of social workers and medical providers bridges the gap between historically siloed medical and mental health fields.
Required Qualifications
CSW I : Master's degree in social work, psychology or related field from an accredited university and knowledge and abilities essential to the successful performance of the duties assigned to the position.
CSWII : Master's degree in social work, psychology or related field from an accredited university and possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science; and knowledges and abilities essential to the successful performance of the duties assigned to the position.
