Position Objective :
The STH Outreach Professional represents Horizon House in providing high-quality, housing-focused outreach support to households referred by the Streets to Home Initiative. Services are delivered using a Housing First, trauma-informed, equity-centered approach, consistent with Continuum of Care policies.
The position is accountable to clients, Horizon House, and the Streets to Home team. Accountability occurs within the context of ethical standards, organizational policies and procedures, Professional Blended Street Outreach (PBSO) standards, and contractual obligations.
This position is 100% funded through a contract between CHIP and Horizon House and is dependent on execution and renewal of said contract or securing other funding at the conclusion of the contract with CHIP. Nonrenewal of the contract or lack of a contract for replacement funding will result in termination of the Street Outreach Professional STH position.
Requirements :
- Ability to work a flexible schedule (Start times as early as 7 : 00 am, working more than 8 hours on some days).
- Ability to thrive in a fast-paced, team-based environment with high levels of accountability.
- Frequent travel by foot, car or other means necessary to fulfill job expectations.
- Ability to establish rapport and develop professional working relationships with individuals experiencing homelessness in environmental settings required.
- Ability to demonstrate cultural sensitivity and perspective, respect for clients' self-determination, and focus on client-centered care required.
- Ability to maintain professional boundaries, communication and confidentiality required.
- Proficient use of technology (Microsoft, Google, electronic database) required
- Bachelors degree in social work or related field, or a combination of lived experience and relevant work history
- Previous experience in homeless services preferred.
- Bi-lingual (English / Spanish) a plus.
Special Requirement :
MUST HAVE a valid Indiana operators license in good standing and proof of state-required auto insurance. Utilization of personal vehicle for travel within the community may be required. Approved mileage is paid at current IRS reimbursement rate.Essential functions : In addition to reporting to a Horizon House supervisor, the STH Outreach Professional works with the Director of STH, a CHIP employee, to :
Co-conduct outreach in locations throughout Marion County, as determined by the Streets to Home Leadership CommitteeWork with unsheltered clients in encampments / other locations to build trust and offer services and housing according to the Streets to Home modelAssist clients with gathering information and completing paperworkLearn all relevant processes in the Homeless Management Information System (HMIS);Accurately enter data in HMIS in compliance with all project and agency guidelines, and provide feedback to improve the feasibility of an ongoing outreach data entry processActively participate in regular (daily and weekly) Streets to Home meetings and work groups as requestedComplete the Coordinated Entry System (CES) Navigator Training and conduct CES Assessments to facilitate housing placement for clients.Adhere to the most up to date version of the Coordinated Entry Policies and Indianapolis Continuum of Care (CoC) Standards of CareGeneral responsibilities of the Street Outreach Professional include :
Support individuals experiencing homelessness through direct client contacts, advocacy and empowerment, and collaboration and coordination of services with community providers.Maintain timely and accurate documentation in compliance with project and agency standards.Participate in agency meetings, training sessions, and other development activities as assigned.Accept responsibility for professional competency as appropriate to their discipline.The preceding essential function, general responsibility, and qualification statements are not intended to be an exhaustive list of tasks and functions for this position.
