HH Physical Therapist
JOB SUMMARY
Plans and administers medically prescribed physical therapy treatment for patients suffering from injuries, or muscle, nerve, joint, and bone diseases, to restore function, relieve pain and prevent disability.
DUTIES AND ESSENTIAL FUNCTIONS
Home Health positions providing direct patient care in the patient home will maintain the following :
keep their assigned Home Health provided cell phone with them and turned on at all times during their scheduled shifts
keep their assigned Ever-Bridge phone application on and utilize in accordance to Home Health guidelines
keep their assigned Safety Connection badge charged and with them at all times during their scheduled shifts and will utilize Safety Connection badge in accordance to Home Health guidelines
Reviews physician's referral and patient's condition and medical records to determine therapy treatment required. Confers with physician and other practitioners to obtain additional patient information, suggests revisions in treatment program and integrates physical therapy treatment with other aspects of patient's healthcare.
Tests and measures patient's strength, motor development, sensory perception, functional capacity and respiratory and circulatory efficiency and records findings to develop or revise treatment programs. Documents treatment program with anticipated goals in EPIC.
Supervises patient treatment / programs of the Physical Therapy Assistants and Technicians.
Administers manual exercises to improve and maintain function. Instructs, motivates and assists patient to perform various physical activities.
Evaluates, fits, adjusts and instructs in use of assistive devices such as crutches, canes and prostheses or other orthotic devises.
Evaluates effects of treatment at various stages and adjusts treatments to achieve maximum benefit. Records treatments, response and progress in patient's chart or enters information into the computer. Thoroughly completes all required documentation legibly, accurately and on time.
Instructs patient and family in treatment procedures to be continued at home.
Collaborates with other members of the healthcare team to identify needs and continued services once discharged from current level of care.
EDUCATION / LICENSE / CERTIFICATION / OTHER REQUIREMENTS
Minimum requirement :
Bachelor’s Degree or Doctorate (DPT) in Physical Therapy
Maintains active licensure in the States of Kentucky and Ohio as a Physical Therapist
Achieves and maintains American Heart Association Basic Life Support (BLS)
Current Driver’s License and daily access to a reliable vehicle.
Preferred qualifications :
Two years of experience as a Physical Therapist
WORKING ENVIRONMENT
Constantly works near moving mechanical parts
Constantly exposed to blood and / or body fluids and infection.
Occasionally exposed to toxic or caustic chemicals
Occasionally exposed to risk of radiation
The noise level in the work environment is usually moderate.
Constantly required to operate a motor vehicle long distances in urban and rural areas.
Occasionally exposed to unpleasant home environments
Occasionally exposed to adverse weather and unsafe road conditions
PHYSICAL DEMANDS
Occasionally required to maintain in a stationary position (standing or sitting).
Frequently required to walk and move about.
Frequently required to operate a computer and other equipment.
Frequently required to use fine motor skills to handle and operate instruments / devices.
Constantly required to lift and / or move items up to 10 pounds.
Frequently required to lift and / or move items up to 50 pounds.
Occasionally required to lift and / or move items up to 100 pounds.
Constantly required to communicate with patients and other care team requiring talking and hearing.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
Occasionally required to position self in order to meet patient needs.
