Description :

Review the credentialing checklist prior to submission.

Complete state license verification.

Submit a current resume.

Provide three professional references.

Conduct a county criminal background check covering the last seven years for all counties lived and worked.

Conduct a federal criminal background check covering the last seven years or more.

Perform a multi-state criminal search covering the last seven years or more.

Conduct a nationwide sex offender search covering the last seven years or more.

Complete HHS / OIG search.

Perform GSA search.

Conduct a social security number trace.

Provide proof of a one-step PPD test or Quantiferon test or proof of positive PPD with chest x-ray.

Complete SAM search.

Provide negative drug screen results.

Submit a physical exam report.

Provide a LA Fire Safety Card (renewed every four years).
