Description

Purpose : The Habilitation Counselor (HC) will align his / her scope of work with Jawonio's Mission, while incorporating Jawonio's Core Values as the guiding principle for all work activities.

Summary : To perform direct care activities for individuals. Assists individuals in maintaining good daily living skills and acquiring independent living skills. Provides direction and oversight to individuals who may display challenging behaviors. Serves as a role model and mentor, demonstrating desired behaviors, and coaches using approved techniques and strategies; uses approved physical intervention techniques when necessary to protect individuals and others.

Essential Duties and Responsibilities

  • Encourages and supports assigned individuals to reach their independence through prescribed tasks and documents progress.
  • Performs QIDP duties including case management, assessment, and advocacy to ensure well-being and quality of life.
  • Performs personal care activities based on the individual's capabilities, including grooming, feeding, washing, moving, lifting, and transferring.
  • Consults with supervisor for planning and coordination of activities, development and implementation of goals, including community integration and personal development; documents all activities.
  • Participates in discussions about individuals' care and progress, offers suggestions, and maintains records.
  • Transports individuals to various activities and appointments, following safety procedures and completing vehicle safety checks.
  • Performs cleaning functions throughout the residence, including dining, kitchen, bathrooms, and living areas.
  • Maintains residence operation functions like fire alarm checks and communication books.
  • Prepares meals considering dietary needs, assists during mealtimes, and plans meals within dietary guidelines.
  • Attends mandated meetings and training sessions.
  • Cooperates with investigations or inspections, providing verbal and written statements as needed.
  • Dispenses medication following agency policies and certification.
  • Works overtime as needed, including before and after scheduled shifts, adhering to policies.
  • Performs other duties as required.

Qualifications

Education / Experience : Bachelor's degree in Education, Psychology, Sociology, or Social Work. No waivers permitted.

Language Ability : Able to read, interpret documents, write reports, and speak effectively before groups.

Math Ability : Able to perform basic arithmetic and interpret graphs.

Reasoning Ability : Apply common sense to carry out instructions and solve problems.

Computer Skills : Knowledge of MS Word, Excel, and internet software.

Certificates and Licenses : No certifications needed at hire; must complete DSP training within 24 months.

Supervisory Responsibilities : No official supervisory duties but acts as a mentor.

Work Environment : Moderate noise, occasional exposure to blood / body fluids, outdoor weather, and physical demands including lifting up to 50 pounds.

Additional Requirements :

  • Flexible scheduling, including shift adjustments.
  • Valid driver's license preferred.
  • Maintain current training and clearances as required by NYS Justice Center, OPWDD, and Medicaid.
