Case Manager - Civic Center Apartments

The primary role of the Residential Services Case Manager is to provide brokerage of community resources, conduct one-on-one case management, on-going assessment of clients' progress and needs, advocacy, and implementation of group activities for a formerly homeless, disabled population of single adults. The ultimate goal is to assist these individuals in maintaining their housing and to provide the services, linkages, and interventions necessary to prevent them from returning to homelessness.

$20.64 - $23 an hour WHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVANT YEARS OF EXPERIENCE
