Position Overview :

We are seeking a dedicated and compassionate Physician specializing in Family Practice (FP), Internal Medicine (IM), or General Practice (GP) to join our team in Deerfield, IL. The ideal candidate will provide high-quality care to geriatric patients, primarily aged 40 and up, in Skilled Nursing Facilities (SNFs). This role involves routine monthly visits to ensure continuity of care, timely updates to care plans, and collaboration with SNF staff. Minimal hospital rounding is required for patients needing acute attention.
