POSITION OBJECTIVE / SUMMARY :
Under the direct supervision of the Program Director, Adult Out-Patient Services. Responsible for the navigation of Agency admissions to the community outreach, intensive outpatient and crisis services on behalf the Agency’s services in collaboration with the Los Angeles County Department of Mental Health, and other Agency / Community based services. Provide outreach, engagement, and education to clients, families, and community-based Agencies to enhance access to the mental health system.
MINIMUM QUALIFICATIONS REQUIRED FOR THIS POSITION :
- Bachelor or Master’s Degree from an accredited college in Social Work, Human Services, or a behavioral science preferred.
- Previous experience in a psychiatric healthcare environment, community services or related field.
- Proficiency in a windows computer software environment (MS Word / Excel) helpful.
- Must be organized and detail oriented.
- Good verbal and written communication, interpersonal skills, and the ability to interface with all levels of management and associates while building strong relationships.
- Must have the ability to problem solve, meet deadlines and follow through on work assignments in a fast-paced environment.
- The ability to work overtime and / or additional days on special projects as needed.
GENERAL STATEMENT OF FUNCTIONS :
Responsible for the navigation of admissions to the community outreach, intensive outpatient and crises services in collaboration with Los Angeles County Department of Mental Health, and other agency / community based services.Provide outreach, engagement, and education to clients, families, and community-based agencies to enhance access to the mental health system.Participate in the development of programs to implement the agency’s services in the mental health service community.Develop placement resources for clients and coordinate placement planning with appropriate agencies.Act as liaison between the agency and the Los Angeles County Department of Mental Health and other related agencies regarding services / resources requirements for clients.Initiate reviews and investigate programs of other agencies within the mental health community to ensure the appropriate services are available to clients.Represent the agency in required community based service related meetings.Practice confidentiality in all client and management related matters.Assist in preserving the agency’s mission, goals, and objectives; and help management in establishing its internal service delivery goals and objectives that are consistent with the organization’s public service mission statement.Participate in the fundraising activities of the Agency.Assist in other duties and special projects as assigned.Competencies :
