Certified-NP for Home Health Assessments Urgent Need : As soon as credentialed or by October 15 - end of year Schedule : 8 hour days, 5 days a week (M-F or W-F example) Hours of operation 8 AM - 8 PM (8 hour days only, this includes chart and travel time) Scope of Practice / Responsibility : Visit Type A. Level 4 Comprehensive Home Annual — longer visit : Full annual wellness labs / diagnostics Assess Social Determinants of Health; document needs / recommend resources Care Guides convert signed charts into a 30-day community resource for the member Visit Type C.
