Position Summary : Our Client is seeking an Case Manager RN is responsible for coordinating and managing patient care to ensure quality, cost-effective outcomes. This role involves assessing patient needs, developing individualized care plans, collaborating with healthcare providers, and ensuring continuity of care across various settings. The Case Manager advocates for patients and families while promoting efficient use of healthcare resources.
Key Responsibilities
Assess patients’ medical, psychological, and social needs.
Develop and implement comprehensive care plans.
Coordinate services among physicians, nurses, therapists, and community resources.
Monitor patient progress and adjust care plans as needed.
Educate patients and families about treatment options and available resources.
Ensure compliance with clinical standards and regulatory requirements.
Document all case management activities accurately and timely.
