Overview
As a Care Coordinator, you will play a vital role in managing patient Plans of Treatment (POTs) and ensuring compliance across our areas of operation. This position is primarily focused on treatment documentation, physician authorization follow-up, and regulatory accuracy, while also supporting nursing staff scheduling as needed. Your ability to track deadlines, communicate across teams, and maintain organized workflows will directly impact patient care and operational success.
Responsibilities
Plan of Treatment (Primary Function)
Track, monitor, and manage patient Plans of Treatment (POTs), ensuring timely submission, renewals, and compliance.
Review POT documentation for accuracy, completeness, and regulatory alignment.
Communicate with physicians, pharmacies, and RNs to resolve missing signatures, discrepancies, or documentation delays.
Maintain organized systems for tracking POT expirations and proactively alert staff to upcoming deadlines.
Support audit readiness by maintaining accurate and compliant records.
Staffing Coordination (Secondary Function)
Assist with nurse scheduling to align staffing coverage with patient treatment needs.
Communicate with PRN, traveler, and floater nurses to maximize resource utilization.
Monitor staffing changes related to new referrals and urgent cases.
Collaboration & Communication
Serve as the primary liaison between clinical staff, pharmacies, and leadership for all POT-related processes.
Provide leadership with regular reports on POT compliance, renewal timelines, and staffing alignment.
Partner with HR / recruitment teams on identifying staffing needs related to treatment demand.
Requirements
Previous experience in healthcare administration, clinical coordination, or infusion / home health operations.
Strong organizational skills with proven ability to track deadlines and manage compliance-heavy processes.
Familiarity with infusion therapy, medical terminology, and home health compliance standards.
Proficiency with EMR / EHR systems (AlayaCare preferred) and documentation workflows.
Excellent communication skills for working with physicians, pharmacies, and nursing staff.
