Growing together.\n\nThe Case Manager RN plays a crucial role in the coordination of patient services through an interdisciplinary approach, blending clinical and psychosocial strategies. This position focuses on effective case management to ensure timely patient care at the right stage and setting, aligned with CMS guidelines and seamless collaboration among all involved parties. Discharge planning begins at or before admission and is continually assessed with the goal of optimizing patient outcomes. Monitoring of quality and risk management issues is also essential.\n\nLocations : You may be required to support the following hospitals : \n\nWaukesha Memorial Hospital - 725 American Ave, Waukesha, WI\n\nOconomowoc Memorial Hospital - 791 Summit Ave, Oconomowoc, WI\n\nMukwonago Hospital - 240 Maple Ave, Mukwonago, WI\n\nSchedule : Per Diem, requiring one weekend of availability per month.\n\nTraining : Enjoy two weeks of paid training during standard business hours, Monday to Friday.\n\nPrimary Responsibilities : \n

  • Lead patient disposition and utilization considerations.\n
  • Facilitate collaboration across interdisciplinary teams.\n
  • Effectively solve problems and seek resolutions.\n
  • Communicate directly with staff, physicians, patients, and families.\n
  • Model leadership qualities through respect and efficiency.\n
  • Coordinate care to achieve quality outcomes and manage resource utilization.\n
  • Educate patients and families, promoting continuity of care.\n
  • Engage in multi-disciplinary team conferences for complex cases.\n
  • Communicate daily with patient care staff about assessments and planning.\n
  • Work independently while organizing tasks for maximum efficiency.\n
  • Adjust schedules to enhance communication and meet patient needs.\n
  • Coordinate discharge planning for all age groups.\n
  • Assess post-hospital service needs for complex cases.\n
  • Educate patients / families about care options and ensure paperwork completion.\n
  • Facilitate timely completion of discharge orders.\n
  • Collaborate with various stakeholders for optimal discharge plans.\n
  • Develop care plans for frequent Emergency Department patients.\n
  • Adhere to federal and state regulations and understand managed care principles.\n
  • Share relevant information with the next level of care staff.\n
  • Communicate financial and care issues with the department director.\n

  • Associate's Degree in Nursing or higher.\n
  • Unrestricted RN license in the state of employment.\n
  • Three or more years of experience in a hospital setting, particularly in medical / surgical units.\n
  • Three or more years of inpatient hospital case management experience.\n
  • Three or more years of knowledge relating to community resources and policies.\n
  • Three or more years of clinical experience across all age groups.\n\nPreferred Qualifications : \n
  • Bachelor of Science in Nursing.\n
  • Case Management Certification.\n
  • Strong analytical, critical thinking, and organizational skills.\n
  • Familiarity with Utilization Review and Medicare processes.\n\nThe salary for this role ranges from $28.61 to $56.06 per hour, based on full-time employment factors such as local labor market conditions and experience.
