The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Intake / Discharge Coordinator facilitates the intake, discharge, and housing navigation of residents temporarily housed in Coalition programs.

    Essential Job Functions

  • Acts as the point person for the day-to day operations Transitional Housing Intake and Discharge System; ensures smooth coordination of daily intake process receiving programs and property management.
  • Understands contractual requirements and works with Program Manager to ensure Intake / Discharge Operations are meeting all targets.
  • Identifies goals and strategies to better conduct integrated care and / or VA intake procedures, better serving the needs of homeless individuals.
  • Communicates all process changes in Intake / Discharge arena to other CCH Programs, Partner Agencies, Funders, and other community stakeholders.
  • Partners with Program Manager to complete data delivery to the appropriate funding, such as City of Denver, MDHI, VA and other investors and community partners.
  • Provides ongoing housing case management and navigation services in conjunction with case management staff.
  • Support crisis response and work variable hours as needed.
  • Ensure timely & accurate recording of all clinical / non-clinical services, communications, and tasks in the EHR and other systems as required while maintaining complete up-to-date charts using the system and any future upgrades as directed.
  • Performs other job duties as assigned.
  • Education, Experience and Job Competencies
  • Bachelor's degree in social work, counseling or similar field preferred.
  • Experience working with vulnerable populations or public health highly preferred.
  • Basic understanding of addiction and recovery processes.
  • Understanding of issues related to homelessness including common physical & mental health conditions, recovery-oriented, evidence-based interventions, complexities within provided services, and trauma-informed care.
  • Collaborates cross-departmentally with Coalition staff to grow & contribute to the success of program objectives, culture, & a positive working environment.
  • Ability to build and maintain internal and external relationships that motivate, guide, and / or reinforce the performance of others toward goal accomplishments.
  • Approaches tasks with thoroughness and consistency and holds value for precision and thoughtful review of work to ensure strong outcomes.
  • Must be able to communicate information clearly and effectively through multiple mediums to diverse audiences.
  • Must be able to remain calm, think clearly, and apply sound judgement in high-pressure situations.
  • Must be able to access and interpret information from computer screens and printed materials.
  • Must be able to manage multiple projects in a dynamic environment with respective stakeholders.
  • Must be able to adhere to process protocol.
  • Qualifications Summary

  • Sound critical thinking and decision-making skills.
  • Knowledge of homeless issues and demonstrated sensitivity to and a desire to work with under served populations.
  • Ability to take initiative to implement projects and pursue goals with minimal supervision.
  • Education and Work Experience

  • Must have a valid driver's license.
  • Microsoft Excel or other spreadsheet program experience is a requirement.
  • Bachelor's degree or relevant experience in a related field is preferred.
  • Experience serving homeless community or public health sector preferred.
  • Excellent organization skills and willingness to work flexibly under pressure.
  • $23.22 - $27.85 an hour
