Job Title : Contingent Speech-Language Pathologist (Home Health)
Summary :
Provides professional speech and audiology diagnostic and therapeutic services to clients as prescribed by a physician, in compliance with state practice acts and licensure requirements. Coordinates care, develops individualized treatment plans, and promotes client independence through effective communication and rehabilitative techniques.
Key Responsibilities :
Assess, plan, and deliver speech-language pathology services in home health settings
Develop and revise care plans in collaboration with physicians and clients
Educate clients and caregivers on treatment and recovery techniques
Document clinical findings, progress, and updates in accordance with agency policy
Participate in quality improvement and interdisciplinary coordination
Supervise HHAs and mentor new staff as assigned
Minimum Qualifications
Pathology granted by the American Speech and Hearing Association; or, meets the educational
requirements for certification and is in the process of accumulating the supervised experience required for
certificate.
