A company is looking for a Community Health Worker.

Key Responsibilities

Perform administrative duties including member outreach, data entry, and follow-up tasks

Identify and process referrals for eligible programs and services for members

Collaborate with staff and medical providers to ensure quality service coordination

Required Qualifications

High School Diploma / GED (or higher)

2+ years of clerical or administrative work experience

1+ year of experience with community resources and values

Intermediate computer proficiency, including Microsoft Outlook and Teams

Access to high-speed internet and a designated quiet workspace at home
