Location : Village on the Park Stonebridge Ranch, an Assisted Living with Memory Care senior commnuity
Pay Rate : $30.00-$32.00 / hour
Essential Duties and Responsibilities :
Administer medications safely and accurately per physician orders and regulatory standardsOversee medication aides and conduct monthly medication reconciliation and auditsCoordinate daily staff assignments to ensure consistent resident careCommunicate effectively with physicians, families, and care teams regarding resident needsProvide direct care including ADLs : bathing, dressing, grooming, toileting, mobility, and incontinence supportRespond to medical and behavioral emergencies with sound clinical judgmentComplete clinical documentation including assessments, evaluations, and service plansDocument incidents and communicate promptly with leadershipMonitor care team documentation for accuracy and completenessSupport orientation and ongoing training of care staffAssist with scheduling in collaboration with leadershipAttend required in-services and continuing educationMaintain professional appearance and hygiene-Promote respectful and effective communication across teams and families\Perform other duties assigned to support resident care and operational excellenceQualifications :
Current and valid LVN / LPN license in the state of employmentBasic Life Support (BLS) / CPR certificationMinimum 6 months to 1 year experience in Assisted Living or Long-Term CareProficient in Electronic Health Records (EHR) and basic computer skillsStrong verbal and written communication skillsDemonstrated critical thinking and problem-solving abilitiesKnowledge of HIPAA regulations and compliance standardsAbility to respond with patience and understanding to dementia-related behaviorsPhysical ability to perform essential job functions with or without reasonable accommodationEOE / M / F / D / V
