Position Description :

The Social Services Practitioner coordinates the care and services of patients identified in need of intervention across the continuum of care. The individual acts as a liaison between patients, family, healthcare team and outside community agencies. Develops, facilitates, and executes comprehensive plan of care based on going assessments. Also, specializes in the coordination of care for patients with complex psycho-social issues.

Qualifications

Job Duties / Responsibilities Requirements :

• Medical Social work and / or gerontology desired
• Master's degree in Social Work required.
• Written and verbal communications, interpersonal skills.
• Basic organizational and computer skills.
• LC SW preferred
• LPS Certification after hire
• CPI is required within 30 days of hire
