job_description.job_card.job_descriptionPhysical Therapist II assesses individual patients' Physical Therapy needs and establishes and implements treatment programs. Reviews medical history, applies screening tests, and meets with patient and family to determine clinical needs, impairments, functional limitations, and disabilities. Determines plan of care including interventions, therapy intensity and duration, and discharge recommendations. Provides education to patient, family, and / or caregiver regarding activity purpose, equipment use, home exercise, and activity. Performs regular re-examinations, measures outcomes, and seeks feedback from patient, family, and interdisciplinary medical team. Oversees clinical work of Physical Therapist Assistant and Rehabilitation Aide. Manages multiple patients simultaneously and provides care in multiple clinical settings and locations. Provides timely and accurate services and documentation in accordance with professional and regulatory standards. Participates in continuing education and incorporates new information into practice. Assists with educating and orienting new staff, acts as clinical instructor for students and interns, and participates as instructor in established classes or programs. Participates in educational events within the clinic and community. Attends specific clinical education coursework meeting Physical Therapy Board of California guidelines and provides in-services to staff.
