Now Hiring : Nurse Practitioner (NP) / Physician Assistant (PA) – Post-Acute Care

Locations :  Mesa, Gilbert, and Chandler, Arizona

Schedule :  Part-Time (2–3 Days / Week, flexible weekdays with occasional weekend rotation) | Full-Time Potential

Practice Setting :  Skilled Nursing Facilities, Assisted Living, and Independent Living

Employment Type :  1099 Independent Contractor

Job Summary :

We are looking for a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a skilled nursing facility. In this role, you’ll work closely with physicians, nursing staff, and other healthcare professionals to provide high-quality care to patients in a post-acute setting. Responsibilities :

  • Conduct comprehensive health assessments and develop individualized care plans for patients in a skilled nursing facility.
  • Diagnose and manage common acute and chronic medical conditions in post-acute and long-term care patients.
  • Order and interpret diagnostic tests, including lab work, imaging, and EKGs.
  • Prescribe medications, monitor treatment plans, and adjust therapy as needed.
  • Provide patient and family education regarding diagnoses, treatment plans, and preventive care strategies.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and nursing staff, to ensure coordinated patient care.
  • Perform minor procedures as needed within the scope of practice.
  • Document all patient interactions, assessments, and treatment plans in the electronic medical record (EMR) system.
  • Refer patients to appropriate specialists and coordinate follow-up care as necessary.
  • Maintain compliance with state and federal regulations, facility policies, and best practices in post-acute care.

Requirements :

  • Active Nurse Practitioner (NP) or Physician Assistant (PA) License in Arizona.
  • Completion of an accredited NP or PA program in the U.S. (excludes Puerto Rico).
  • 1-2 years of experience in a post-acute care or skilled nursing facility setting preferred.
