QIDp Program Director

Addison Healthcare Center

Masury, Ohio

At Addison we strive to provide quality support and services to individuals with developmental disabilities within our ICF. Responsibilities may include but not limited too :

  • Maintain a current knowledge of all state and federal rules, laws, and regulations pertaining to operation of an ICF (intermediate care facility).
  • Ensures proper Individual Plan implementation at day programming (ISP).
  • Management of appointments with outside providers.
  • Accurately complete Individual Assessment forms quarterly and maintain supporting documentation.
  • Conduct annual and special Individual Service Plan meetings for each resident on your caseload.
  • Complete all Individual Service Plans, Comprehensive Functional Assessments, Quarterly Reviews.
  • Supervision of the staff assigned to the ICF.
  • Other duties as assigned.

Requirements for this position include :

  • To qualify as a QIDP, you must have a Bachelor's degree or higher in a human services field, such as social work, psychology, education, nursing, or rehabilitation. Your degree program should include courses that cover topics such as human development, disability studies, behavior analysis, counseling, and ethics.
  • One year of experience in the field of developmental disabilities.
  • Supervisory or management experience.
  • Be able to display strong communication skills and excellent decision-making and time-management skills.
