Position Summary

A managed care organization serving New Hampshire Medicaid members is seeking a Licensed Practical Nurse (LPN) to join its Care Management team in a telephonic role. This position begins as a temporary assignment with the opportunity to transition to permanent employment based on performance and organizational needs. The LPN will support care coordination, admissions, and member engagement primarily by phone, with occasional in-person visits to skilled nursing facilities (SNFs) or members' homes as needed (mileage reimbursed). The ideal candidate will have experience in long-term care, post-acute intake, or transitional care coordination and be comfortable working remotely in a structured, fast-paced environment.

Key Responsibilities

• Conduct telephonic outreach to members, families, providers, and facilities to coordinate care, admissions, and services.
• Support transitions of care for members entering or discharging from long-term care, SNF, or rehabilitation settings.
• Gather and review clinical documentation, assessments, and eligibility information for RN or physician review.
• Serve as a liaison between the health plan, facility admissions teams, discharge planners, and providers to ensure timely and efficient placements.
• Assist with prior authorization requests, documentation preparation, and Medicaid eligibility verification.
• Educate members and families on benefits, services, and expectations during transitions of care.
• Document all interactions, activities, and clinical notes accurately in the electronic medical record (EMR) and care management platform.
• Escalate complex clinical or social concerns to the RN or Utilization Management team.
• Ensure compliance with HIPAA, state regulations, and organizational protocols.
• Complete occasional field visits to SNFs or members' homes in New Hampshire, with mileage reimbursement.
