General Description of the Job Class

The Oncology Resource Specialist is an integral part of the patient care team and is responsible for navigating oncology patients and referring providers through the new patient referral process and first visits to the Duke Cancer Centers. The specialist will work with patients and referring providers to identify specific needs for their oncology care, including timeliness of the visits, provider coordinator and psychosocial care.The Oncology Resource Specialists will work as an integral part of an interdisciplinary team, ensuring excellence in patient care, in an effort to achieve optimal clinical outcomes through a seamless model of access and care.

Duties and Responsibilities of this Level

For new oncology patients, the Oncology Resource Specialist will ensure comprehensive, timely, and seamless care through initiation of the referral process, coordination with medical team members, and providing patient education and support. This position will be the first contact for all new patients coming into the Duke Cancer Centers, as well as the contact for referring providers who are referring new oncology patients into the system.Have a full understanding of ALL cancers, providers, scheduling guidelines and necessary records within ALL oncology disease groups at ALL locations.

Exhibit extensive understanding of patient care and health system pathways and serve as a conduit between patient and providers to address appointment clinical urgency, patient preference around scheduling timeframes and psychosocial needs.Use comprehensive communication and interviewing skills to respond to information requests. Translates technical information to language / literacy level of each customer and uses active listening skills to verify understanding.Along with asking relevant information based on cancer diagnosis, stage, medical history, gender, age and documenting this into the patient chart, the Oncology Resource Specialist will assess educational needs of patients. Provide education about diagnosis, treatment, side effects, and post treatment care or other specialty topic area based on patient population needs. Provide care and education in a culturally competent manner.Independently identify patient's needs, goals and preferences. Ensure those are integrated into the patient's care delivery. Coordinate next day access for patients via telephone, telehealth or in person visit ensuring all new patients are able to have needed rapid access if desired.Document all patient encounters in EPIC. Create and document in disease specific flowsheets for access nurses, NPCs and clinical teams to utilize during the scheduling process and patient visits.Evaluate patients, using approved questionnaires, for support services such as Smoking Cessation, Onco-Fertility, Nutrition and Palliative Care. Make necessary referrals prior to first appointment.Collect medical records from referring providers. Determine appropriate records based on specific requirements by disease group and cancer diagnosis. Make these records available in Epic for access nurses, NPCs and clinical teams. Utilize necessary programs such as PowerShare, Care Everywhere and eHealth.Bridge gaps in care and assess / troubleshoot transitions in care and barriers to new care.Remain the patient's point of contact throughout the entire referral process, ensuring they are prepared for their appointments. Ensure all necessary questionnaires are complete in MyChart prior to the appointment.Provide continual status updates to referring providers throughout the process, including any barriers to care related to insurance, medical records and patient availability.Participate in departmental meetings related to new patient access in Oncology.Maintain liaison with other programs within the Duke Oncology Access department to coordinate program business and to accomplish program objectives.

Minimum Qualifications

Education

Preferred Bachelor's degree in health education or public health fields preferred.

Experience

Three years of experience in a patient-centered, health related field. Two additional years of experience may be substituted for a degree.

Degrees, Licensures, Certifications

Certified Health Education Specialist preferred.
