The Registered Nurse (RN) in the unskilled home health program is responsible for overseeing and coordinating personal care services provided by home health aides and caregivers. The RN conducts initial and ongoing client assessments, develops individualized care plans, supervises caregivers, ensures compliance with regulatory standards, and promotes high-quality, safe, and compassionate care in the home setting.\n\nKey Responsibilities : \n
- Perform initial nursing assessments and develop individualized plans of care for clients receiving personal care or companion services.\n
- Conduct supervisory visits in the client's home to monitor the quality of care provided by aides and ensure compliance with the care plan.\n
- Review and update care plans as clients' needs change.\n
- Educate and train home health aides on care procedures, infection control, safety, and client-specific needs.\n
- Document all assessments, supervisory visits, and care plan updates accurately and in a timely manner.\n
- Communicate regularly with the client, family members, caregivers, and agency staff to ensure coordinated care.\n
- Ensure that all services meet state and federal regulations, agency policies, and payer requirements.\n
- Report any significant changes in client condition or environment to the appropriate Case Manager and agency.\n
- Participate in agency meetings, audits, and quality improvement initiatives.\n\nPhysical & Work Requirements : \n
- Ability to travel to clients' homes and perform visits.\n
- May require occasional lifting or assisting clients.\n
- Work is performed in a variety of home settings.\n\nQualifications : \n
- Education : Graduate of an accredited nursing program\n
- Licensure : Current, unencumbered RN license in the state of practice.\n
- Experience : \n
- Home care experience a plus.\n
- Certifications : Current CPR / BLS certification.\n
- Other Requirements : \n
- Valid driver's license and reliable transportation.\n
- Excellent communication and organizational skills.\n
