Job Description\n

American Traveler seeks an experienced RN for a Home Health float position in the South Region of Massachusetts, with MA RN licensure and certified home care experience required.

\n \n Responsibilities\n

  • Work in home health settings covering Dorchester, Jamaica Plain, Mission Hill, and Roxbury
  • Home Health assignments float between Paul Revere and Braintree areas as assigned
  • Day shift schedule from 08 : 00 to 16 : 30
  • Weekend rotation required approximately once every 6 weeks
  • Patient care and case management responsibilities using EPIC EMR
  • Travel required to patient homes for assigned cases
  • Responsibilities include patient assessment, care plan implementation, patient and family education, admissions, and timely electronic documentation
  • Adheres to community health and organizational standards of practice
  • Works in collaboration with an interdisciplinary team
  • EPIC charting system is used for documentation
  • Local candidates must reside at least 60 miles from central Boston for travel pay; otherwise, local rates apply
  • Parking at Boston locations is not validated and incurs daily costs; public transportation options are available
  • Timekeeping is managed through UKG / Kronos system
  • Holiday rotation and additional weekend coverage may be required as determined by the department

  • Active MA RN license required; conditional or pending licenses not accepted
  • Minimum 1 year of certified home care experience required
  • At least 1 year of case management experience in a home health setting
  • AHA certifications required
  • Must provide primary source verification of professional license and certifications
  • Proof of MA licensure required for consideration
  • Flu vaccine required with no medical or religious exemptions accepted
