Work Schedule :
100% FTE; 50% AM shifts (7 : 00 AM - 3 : 30 PM); 50% PM shifts (1 : 30 PM - 10 : 00 PM). 1 in 4 weekends and holiday rotation required. Hours may vary based on the operational needs of the department.

We are seeking a Clinical Pharmacist to :
Provide medication therapy management services as part of the interdisciplinary team to patients on inpatient services to assure that drug therapy is safe, effective, efficient, and cost-effective.Perform both clinical and distributive tasks necessary to ensure that patients' medication therapy is safe, effective, timely, and efficient.Provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences.Provide patient care to a diverse population including medical and surgical patients.At UW Health, you will have :

Bachelor's Degree in Pharmacy or a PharmD degree RequiredCompletion of an ASHP Accredited Residency training program PreferredWork Experience
1 year of clinical patient care and orders management experience Required3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program PreferredLicenses & Certifications
Licensed to practice pharmacy in the State of Wisconsin within 90 Days RequiredBasic Life Support (BLS) within 90 Days RequiredCertification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) PreferredOur Commitment to Social Impact and Belonging
