Experienced MDS Coordinator in LTC Setting

Avir Health Group is seeking a qualified MDS Coordinator to join our family!

On Call rotation once a month

EMR : PCC

We are searching for a Licensed MDS Coordinator who is resident & family focused, a team builder, and excited about the opportunity to build and lead a high performing team and culture.

Qualifications :

Valid Texas Nurse License.

Valid CPR Certification.

Minimum of one year of experience in a supervisory capacity in a hospital, skilled nursing facility, or other related healthcare program.

Must possess knowledge of long-term care operational standards as set forth in the Federal Register, Conditions of Participation and State regulations.
