Job Description

Position : Care Coordinator Bilingual preferred

Location : Remote

Status : Full-time and part-time available

Summary :

We are seeking a compassionate and organized Care Coordinator to join our team. This individual will be responsible for identifying social determinant barriers and / or providing resources for needs that are identified. This may include, but not limited to, locating and coordinating programs and services in local communities or through National Programs to help improve the quality of overall health and wellness.

Responsibilities :

Advocate for members to ensure they receive appropriate services and support
Monitor and evaluate member progress and adjust care plans as needed
Provide education and support to members
Develop and maintain an accurate and thorough knowledge of all program requirements
Stay current on healthcare trends and best practices

Qualifications :

Experience in healthcare, customer service, social work or nursing preferred

Excellent time management, organizational, and communication skills

Ability to work independently and as part of a team

Must comply with company credentialing requirements

Must have a good working computer equipped with high-speed internet, and the ability to keep computer secure per HIPAA guidelines.

Excellent problem-solving and critical thinking skills

Proficiency in Microsoft Office
