Description :

The Home Health Visit Nurse works collaboratively with the Home Health RN Care Manager and other disciplines.

Provides skilled care, education, treatment, and emotional support to patients.

Monitors and assesses the care provided to ensure that patients attain and maintain their highest level of functioning.

Ensures that the Physician and all members of the care team are aware of pertinent changes in the patient’s condition and progress toward goals.

Collaborates with the Home Health RN Care Manager to ensure the established plan of care is appropriate and implemented or modified based on patient responses to treatment.

Essential Functions :

Provides skilled nursing care to an assigned caseload of patients.

Monitors the care, treatment, and service provided by other disciplines to ensure patients receive the required care to improve their health status.

Educates patients and their families on proper home health care strategies and procedures.

Instructs and supervises patients and families in the care of the patient and maintenance of a healthy environment.

Evaluates and revises the plan of care in collaboration with the Home Health RN Care Manager as appropriate.

Reviews the plan of care of assigned patients to ensure that physician orders are followed and care is provided to achieve established goals.

Performs skilled assessments and documents findings in the OASIS accurately and timely.

Maintains an accurate and current list of each patient’s medications.

Communicates regularly with the Home Health RN Care Manager regarding patient progress.

Establishes positive working relationships with physicians and keeps them informed of patient problems and progress.

Assesses patient outcomes and offers suggestions for improvement.

Participates in the performance improvement plan to ensure positive outcomes.

Actively participates in interdisciplinary team meetings.

Participates in on-call rotation as required.

Physical Factors :

Ability to lift 50 pounds, turn, and position patients weighing over 200 pounds.

Ability to stand, walk, kneel, sit, crouch, or stoop with good balance for extended periods.

Must be able to maneuver over a variety of uneven surfaces.

Good manual finger dexterity and communication skills are required.

Must be licensed to operate a motor vehicle.

Must tolerate various environments, including the presence of pets.

Physical presence onsite is essential.

Hearing and vision must be normal or corrected to within normal range.

Able to perform duties with or without reasonable accommodation.
